EB, a 42 y/o female, is admitted with chest pain radiating to her left arm and jaw. She states the pain began while she was on the couch watching TV and coincided of a feeling that her heart was racing. Pt is thin, eyes noted to be slightly bulging, slight tremor in hands. Pt reports recent weight loss, anxiety, and difficulty sleeping. Admitting vitals include HR of 132 and B/P of 169/91. Troponin normal. EKG shows sinus tachycardia. Chest pain only improved mildly with nitrates but resolves with beta blocker administration. Cardiac catheterization shows no blockage in coronary artery. The provider informs her that her chest pain was likely the result of demand ischemia secondary to tachycardia, although they are still investigating the cause of her tachycardia and hypertension. Pt’s HR and B/P remain moderately elevated even on beta blocker therapy. Provider ordered additional bloodwork. Significant findings: T4 – 23.7 pmol/L, T3 – 7.8 pmol/L, TSH – 0.4 mU/L.
