[SOLVED] A new patient calls your office for an appointment

A new patient calls your office for an appointment, what information do you need to collect from them at the time of their initial call?  Why is this information necessary? Now policies that must be followed include:
Verifying the spelling of the patient’s name, and asking for their date of birth
For new patients, searching the EHR database prior to creating a new record
Discuss the importance of these two actions and describe issues that may occur if they are not followed.  Why is it important to complete this PRIOR to the patient coming in for their visit?  What are some issues that can arise if not done before they arrive?
