[SOLVED] The Fiver Children’s Foundation Theory

Based on the Fiver Children’s Foundation Theory of Change and Narrative (located in files) complete the Logic Model template using the steps below. In addition to completing the template (attached), please write a brief narrative outlining your thought process for the logic model.

Steps:

-Begin with the problem/situation/need (in your words, identify the situation/need based on the Fiver model)
-Next plug in the short-term outcome (see short-term outcome below).
-What is the intervention that leads to the outcome? (Mentoring program of some sort; make sure the intervention actually leads to the outcome)
-What are the activities of the intervention? (each activity should be listed separately as you will need to measure them separately)?
-How will you measure (prove) the outputs? (these are activity-specific tools such as rosters showing that mentor/mentee met 2x/week; you are looking at process here)
-How will you measure (prove) the outcome (this should measure beyond the activities and look at the relationships with caring adults; you are looking a behavior change here)
-Who, what, when/how often?
